IMMUNIZATION FORM ( filling in India, Mumbai/Thane)

I received a pdf form PRE-ENTRANCE IMMUNIZATION RECORD FORM.
I am currently in India (Thane, Mumbai ).
Few Doctors I spoke to don't want to fill up this form.

Does anyone know of any Doctor in this area India (Thane, Mumbai ) who would fill up this form ?
